Community Health Worker- Full Time

Position Summary

The position serves as a link between clients who have been affected by COVID-19 and social services.  This includes interviewing, and counseling COVID-19 individuals and families as well as providing referrals. This position provides support to the Health Department in order to help meet their mission of promoting and protecting the health of our community.

 

Accountability

Reports directly to the Project Coordinator. Works as a team with the Local Health Department and with local United Way Staff

 

Essential Responsibilities

  • Serve as support staff at the vaccine clinic and vaccine call center: making appointments, serving as patient traffic control or data entry into CVMS

  • Assist the billing and insurance office with research and data entry for COVID related claims

  • Collect and report data using report forms

  • Screen COVID-19+ patients for symptoms and conduct the pre-tracking interview

  • Counsel individuals and families with CDC recommended isolation procedures, provide referrals and conduct check-ins to monitor progress

  • Assist in the preparation and distribution of education and marketing materials

Qualifications

Minimum of associates degree preferred. 

$20.00 per/hour
